Title	Case Manager

Salary Range:	To $75,000
Education:	"RN, BSN preferred
Location:	Nebraska or Colorado



Job Description:	Responsibilities: Follow up on evaluations, educate the patient, assure the correct tests for the right symptoms and coordinate various efforts for patient care Understand the patient's physical condition, testing procedures and results and medications Act as an patient advocate to encourage proper adherence to treatment plan by addressing patient's needs Help the patient understand the evaluation and follow the treatment plan Review and present cases to the company and health system's CMOs Able to travel to meet with patients and/or their physicians Maintain member's cultural beliefs QUALIFICATIONS: RN, BSN preferred, with license and malpractice insurance to practice in Nebraska or Colorado Minimum of seven (7) years of direct patient care experience Mobilize resources available from community, agency and ancillary services Basic understanding of current utilization review/condition management Broad understanding of healthcare "best practices" Excellent written and oral communication skills Bilingual skills preferred, ie. English/Spanish Proven ability to multitask Enjoy life - it has an expiration date!
